CONDITIONS

This Equivalency Certificate authorizes Purolator Inc., to handle, offer for transport or transport dangerous goods that are UN1845, DRY ICE, Class 9, in a manner that does not comply with Packing Instruction 954 of Chapter 11 of Part 4, and paragraph 4.11 of Chapter 4 of Part 7 of the Technical Instructions for the Safe Transport of Dangerous Goods by Air, as it relates to the keeping of the alternate document identified as an adhesive address label, that includes a routing bar code, incorporated by reference in subsection 12.1(2) and section 3.11 of the Transportation of Dangerous Goods Regulations, if:

(a) a Purolator Inc. "Dry Ice Acceptance Checklist" is completed which includes the following information:

(i) UN 1845

(ii) Carbon dioxide, solid or Dry Ice

(ii) Class 9

(iv) the number of packages

(v) the net quantity of dry ice in each package;

(b) the Purolator Inc., "Dry Ice Acceptance Checklist" is kept as an alternate document for a period of two (2) years after the date the dangerous goods are no longer in transport;

(c) within 15 days after the day on which it receives a written request from an inspector, Purolator Inc., produces a copy of the Purolator Inc., "Dry Ice Acceptance Checklist"; and

(d) the Purolator Inc., "Dry Ice Acceptance Checklist" includes the following information legibly and indelibly printed:

"Equivalency Certificate No. SA 8911 (Ren. 6)"

Note: The issuance of this Equivalency Certificate in no way reduces the certificate holder's responsibility to comply with any other requirements of the Transportation of Dangerous Goods Regulations, the Technical Instructions for the Safe Transportation of Dangerous Goods by Air, the International Maritime Dangerous Goods Code, and the Canadian Aviation Regulationsnot specifically addressed in this certificate.

Explanatory Note

This Equivalency Certificate authorizes Purolator Inc., to transport dangerous goods without keeping the alternate document that is identified as an adhesive address label that includes a routing bar code. The required information identified in Packing Instruction 954 of Chapter 11 of Part 4 of the Technical Instructions for the Safe Transport of Dangerous Goods by Air, is recorded on the alternate document which is the Purolator Inc., "Dry Ice Acceptance Checklist".
