CONDITIONS

This Equivalency Certificate authorizes United Initiators, Inc., to handle, offer for transport or transport, and authorizes any person to handle or transport on behalf of the approval holder, by road vehicle, dangerous goods that are UN3109, ORGANIC PEROXIDE TYPE F, LIQUID, (tert-Butyl hydroperoxide 70% in water), Class 5.2, Packing Group II, in a manner that complies with Packing Instruction 9 of the National Standard of Canada CAN/CGSB-43.146-2002, "Design, Manufacture and Use of Intermediate Bulk Containers for the Transportation of Dangerous Goods", January 2002, adopted by reference in subparagraph 5.14 (1)(a)(i) of the Transportation of Dangerous Goods Regulations if:

(a) the design of the Intermediate Bulk Containers (IBC) remains in compliance with the design on file with the Director, Regulatory Affairs Branch, Transport Dangerous Goods Directorate, Transport Canada, and is a UN 31HA1, manufactured by Hoover Materials Handling Group Inc., under the name "TuffTank II";

(b) the Intermediate Bulk Containers (IBC) are dedicated to the transport of UN3109, ORGANIC PEROXIDE, TYPE F, LIQUID;

CONDITIONS

(c) the Intermediate Bulk Containers (IBC) have a maximum capacity that does not exceed 1 250 L; and

(d) the shipping document that accompanies the dangerous goods contains the following information legibly and indelibly printed:

"Equivalency Certificate No. SH 7511 (Ren. 5)"

Note: The issuance of this Equivalency Certificate in no way reduces the certificate holder's responsibility to comply with any other requirements of the Transportation of Dangerous Goods Regulations, the Technical Instructions for the Safe Transportation of Dangerous Goods by Air, the International Maritime Dangerous Goods Code, and the Canadian Aviation Regulationsnot specifically addressed in this certificate.

Explanatory Note

Packing Instruction 9 of National Standard of Canada CAN/CGSB‑43.146‑2002, "Design, manufacture and Use of Intermediate bulk Containers for the Transportation of Dangerous Goods", January 2002, requires that the dangerous goods be offered for transport and transported in an IBC that is of a design that is approved by the competent authority of Canada.
